Free printable blood pressure logs
Print any of these and start today. There is no email form and nothing to create an account for. Take each reading with a blood pressure monitor and write it down. Paper works perfectly well, and a filled-in sheet is easier to hand to a clinician than a month of memory.
Seven-day log
The one most people want. A week of readings on a single page, with room for the morning and evening pair that clinicians usually ask for.
Download PDFLarge print
The same week, set in much bigger type with wider boxes. Easier to fill in by hand and easier to read back across a table.
Download PDFMonthly log
A full month on one sheet. Better when you are tracking a longer stretch and do not want to carry a stack of pages.
Download PDFWith a medication column
Adds a column for what you took and when, next to the reading. Useful when you and your clinician are looking at the two together.
Download PDFRegistro en español
The seven-day sheet in Spanish, with the same columns and the same layout.
Download PDFSeven-day, low ink
The seven-day sheet stripped of heavy fills, so it costs less ink on a home printer.

- Sit still for five minutes first, feet flat, back supported, arm resting at heart height.
- Write both numbers and the time. The time is what makes a week of readings readable later.
- Take readings at roughly the same times each day, usually once in the morning and once in the evening.
- Write down what you actually got, including the readings you do not like. A log with the bad days removed is not much use to anyone.
Cuff is a place to record readings, not a medical device, and it does not diagnose anything. Your clinician interprets the numbers and sets your target.
Rather not keep track of the paper
Cuff does the same job on your phone. You enter each reading from your monitor and it lands in a history you can scroll or view as a chart. There is no account and no login, and your readings stay on your device. It does not measure blood pressure, so you still need the monitor.
The app is free to use for logging. If you want a PDF report of your history to bring to an appointment, that is part of the paid Cuff Pro subscription. You can look at a sample report before deciding.
